 This seminar is for dogs that are “In Need of Speed”.

Specifically designed for dogs that are having difficulty making course times.

Learn to train “In Drive”.

Learn how to increase your dog’s speed through motivational techniques.

Learn how to develop course handling strategies to improve your team performance.

Seminar limited to 12 working spots.

Seminar Fee: $135.00 (includes snacks and lunch)

8:00am – 4:30 pm at Camp Bandy, Amherst Junction, WI

A $100.00 non-refundable deposit is needed to insure your spot.  The remainder will be due July 25, 2008.

Disclaimer and Waiver of Liability

I hereby acknowledge that I have voluntarily applied to participate in dog training activities with

Cindy Brick at Camp Bandy.   I am aware that there are inherent risks and hazards involved with and around dogs, and I am voluntarily participating in the activities with the knowledge of these potential dangers. I am not relying on Cindy Brick or Camp Bandy to prevent such occurrences. In order to participate in the Cindy Brick seminar I, being fully informed of such risks and hazards, agree to assume ALL RISK OF SUCH OCCURRENCE.

I hereby waive any and all claims or actions I or my guardians, representative or assigns may have against Cindy Brick or Camp Bandy and agree to release Cindy Brick or Camp Bandy from liability in any and all personal injuries to myself, my dog, children in my charge, or harm to property causes directly or indirectly by any acts that might occur in conjunction with dog training classes, or other activities. I also agree to assume sole responsibility for injury or damage caused by myself, children in my charge, or by the dog I own or handle during participation in Cindy Brick seminar or Camp Bandy and further indemnify, defend, and hold Cindy Brick or Camp Bandy harmless from any damage, loss, liability, or expense, including legal costs and attorney fees, which result from damage caused by myself, children in my charge, or the dog I own or handle.

RELEASE OF LIABILITY -- I have carefully read this release of liability and fully understand the contents thereof. I am aware that this is a release of liability and a contract between Cindy Brick and Camp Bandy, and me and I agree to it of my own free will.

Cancellation Policy

Before June 1st: All fees will be refunded with the exception of the non-refundable/non-transferable deposit.

June 2nd- July 31st: 50% of the total fees paid will be refunded with the exception of the non-refundable/non-transferable deposit.

Aug. 1st to the start of camp: 50% of the total fees paid will be refunded ONLY IF YOUR SPOT CAN BE FILLED with the exception of the non-refundable/non-transferable deposit.

CANCELLATION POLICY -- I have read the above cancellation policy and I agree to all terms. I understand that there are NO EXCEPTIONS to the policy regardless of the reason that I cancel my Camp reservation.
